1. Open the Service Authorization PDF file.

2. Save the file to your computer or just print if you wish to forward by US Postal Service.

3. Fill in the form to the best of your ability. Some Adobe Acrobat versions will let you fill in the form on your computer; however, a signature will be required; the form will need to be printed at some point. 4. Once you have the form filled in and wish to use the USPS, enclose the form and payment, check or money order, and forward to:

5. To email your request, you will need to call for an authorization number to pay by a major credit card. For your security we do not authorize credit cards on the internet. Be sure to include your authorization number on your request.

6. Be sure to include the request form as an attachment with your email Most cell phones can take high resolution photos of your Authorization form to use as an attachment. Just ensure the photo is readable before you send it.
